Although 'Coughlan' is an English case, it decides UK law and it is exceptionally unlikely that the Court of Sessions would rule that Scots must pay for services the English get free!
You should adapt the sample 'complaint letters' on www.nhscare.info and add the sentence below.
The complaints procedure is the same:
